The deep plane repositions the structure beneath the skin, so the skin itself is never stretched. That is the whole point of the technique. Face and neck, planned and performed personally by one European Board-certified surgeon.

One face at a time. Your consultation, your operation and your follow-ups are handled personally by Dr. Akçay, never delegated.
Deep plane face & neck lift
If you can see where a lift was done, it wasn’t done well. These are healed faces, and every caption states the stage plainly. One was taken at a single month, a casual photo, still settling. We include that unpolished stage on purpose: honesty about healing is part of the work.

“A pulled face tells everyone what happened. A deep plane lift, done properly, tells no one. I move the layer that has descended and lay the skin back without tension, so what people see is simply you: rested, still recognisably yourself. The dissection that makes this possible takes hours. I do not shorten it, and I do not hand it to anyone.”

The actual documents: fellowship of the European Board of Plastic, Reconstructive & Aesthetic Surgery, specialist certification, medical diploma. Tap to read them.
Verify his fellowship in the EBOPRAS registryDr. Akçay, personally. There is no rotating roster of “experts” here, and no stranger introducing himself in the pre-op room. The surgeon who answers your first message is the one who examines you, performs the operation and studies every healing photo you send afterwards. His European Board fellowship can be checked independently in the EBOPRAS registry linked above.
The windswept look comes from pulling skin. The deep plane is the opposite: the tissue layer beneath the skin is released and repositioned, and the skin is simply laid back without tension. That is why a well-done deep plane lift reads as rested rather than tight. The jawline returns, the neck softens, and you still recognise the person in your old photographs.
It is taken seriously at every age, and more so at yours. Patients over 55 have pre-operative cardiac screening on top of the standard blood panel and ECG, anaesthesia is run by a board anaesthesiology team, and the operation takes place in a full private hospital with an intensive care unit on site, never an office or “clinic”. You spend the first night under nursing care, not in a hotel.
Aftercare does not stop at the departure gate. Your sutures come out before you travel, and Dr. Akçay himself confirms you are fit to fly. From home you send healing photos at weeks 2, 6 and 12, then at months 6 and 12, on the WhatsApp thread you have used since day one. You also carry a complete set of your medical records, written in English, for any doctor who asks.
The retaining ligaments are released and the structural layer beneath the skin, not the skin itself, is moved back to where it sat years ago. Face and neck treated together, in one operation of four to five hours.
Ask about the deep plane liftThe midface lifted from above through small incisions hidden in the hairline, guided by a camera. Less dissection, a quicker recovery, and nothing visible in front of the ear.
Ask about the endoscopic liftFor faces that need repositioning rather than skin removal. Every incision sits in hair-bearing scalp, so there is no scar to find even with the hair worn up.
Ask about the scarless liftA heavy or descended brow returned to where it belongs. It opens the eyes without touching the eyelids, and is often what a “tired face” actually needs.
Ask about a brow liftA focused lift of the outer brow and eye corner through an incision in the temple hair. It is the most subtle change on this page, and for the right face, the only one needed.
Ask about a temporal liftA high hairline brought forward and the forehead shortened, with the incision concealed along the new hairline. Frequently planned together with a brow lift.
Ask about forehead reductionIf you have done your research, you already know the question to ask: does the surgeon truly perform extended deep plane work, with ligament release, platysma and the midface, or a tighter skin pull under a fashionable name? Here is the answer, on camera, from Dr. Akçay’s own operating room.
Surgical footage — viewer discretion
Most clinics show you nothing. We’d rather you see exactly what “deep plane” means: real footage from Dr. Akçay’s operating room. Graphic, honest, optional.
Yes, that is longer than most procedures need. A facelift earns it. Dressings, staged suture removal and a surgeon’s sign-off before any flight are not luxuries here; they are policy.
Clear phone photos: front, both profiles, chin down. Tell him what bothers you when you look in the mirror. Dr. Akçay reviews them personally and you receive an honest assessment, including, sometimes, “not yet.”
A recommendation on technique, whether deep plane, endoscopic, or a combination of both. A written all-inclusive quote that leaves nothing out. A video consultation with Dr. Akçay himself before you commit to anything. Nobody will rush you with deposit demands or expiring offers.
Your driver waits at Istanbul Airport and brings you straight to the recovery hotel near the hospital. The coordinator who has been on your WhatsApp thread since your first message checks in that evening. Surgery is tomorrow, so tonight your only job is to rest.
In-person examination, then pre-operative tests: blood panel, ECG, anaesthesiology review, and cardiac screening for patients over 55. Surgery follows at the ISO 9001-certified Istanbul Aesthetic Center, his partner hospital — a fully equipped private surgical hospital with a dedicated anaesthesia team. A full deep plane face and neck lift takes four to five hours. You stay the night under nursing care.
You rest at your hotel between scheduled in-person visits for dressing changes, swelling checks, medication and daily guidance. A healing face raises new questions every morning. Someone from the team is there, in person, to answer them.
Sutures are removed in stages between days seven and ten. Dr. Akçay examines the result, and only when he signs off are you cleared to fly home, carrying your complete medical records in English.
Photo reviews at weeks 2, 6 and 12, then months 6 and 12, on the same WhatsApp thread. A facelift settles over a full year, and Dr. Akçay watches it settle with you.
The stay is longer because the operation is bigger: you fly home only after Dr. Akçay has examined your face and signed it off — not before, and never to match a cheaper flight. The minimum-stay policy, plainly
Photos first, then numbers. Once Dr. Akçay has reviewed your pictures, your personal written quote normally follows within 24 hours.

Plan for 8–10 nights — deliberately longer than most procedures. A facelift needs dressing changes, staged suture removal between days seven and ten, and a surgeon’s examination before any flight. We state it plainly because it is a safety policy, not a hotel upsell.
Honestly: bruising and swelling peak in the first week, and most patients feel social-ready at around two to three weeks. Earlier with a scarf and sunglasses, later if you bruise easily. The refined result settles over months; that is why the follow-up programme runs a full year.
Along the natural creases in front of the ear, around the earlobe, behind the ear and into the hairline, placed so that for most patients, hair worn down covers the incision lines from the first week, and a healed scar is hard to find even with it up. Endoscopic and scarless techniques move the incisions entirely into hair-bearing scalp.
No operation stops time; your face will continue to age after a lift, like everyone else’s. What surgery changes is the starting point: ageing carries on, but from a visibly younger position. How far that carries you depends on your skin, your bone structure and the life you live, which is exactly why the assessment is personal rather than promised in advance.
Often, yes: one anaesthesia, one recovery is frequently the safer and better-balanced plan, because the upper and lower face age together. Whether a combination is right for you is decided at consultation, never assumed; it changes theatre time, and your written quote will say exactly what is included.
Anatomy decides, not the calendar. Some faces show real jowl and neck laxity in their late forties; some seventy-year-olds are excellent, well-screened candidates. The photo assessment answers this honestly, including, sometimes, that surgery is not the right answer yet.
Because patients who research deep plane surgery have learned to ask whether a surgeon really performs it, with ligament release, platysma work and the midface, or just markets the name. Footage is proof that no adjective can fake. It stays behind a discretion gate, never autoplays, and you never have to watch it.
Surgery takes place in a full private surgical hospital, not an office or “clinic”, with pre-operative blood tests and ECG, cardiac screening for patients over 55, a dedicated anaesthesiology team and on-site intensive care. You spend the first night under nursing care. This is non-negotiable, for every patient.
Surgery and anaesthesia, the hospital night, your recovery hotel for the full 8–10 night stay, all VIP transfers, your coordinator, every post-op check and dressing in Istanbul, medication, and the 12-month follow-up programme. It is all set out in a single itemised written quote, and the total never moves.
A driver, waiting with your name at Istanbul Airport. Every transfer is private and door to door: airport to hotel, hotel to hospital, and back again at the end. Your English-speaking coordinator is already on WhatsApp before you land, so you are never standing in arrivals wondering what happens next.
You message the thread you already have, and your concern reaches Dr. Akçay rather than a help desk. While you are in Istanbul you are seen in person, the same day if needed. Once home, he reviews your photos at every scheduled check, and between checks whenever something worries you. The records you carry in English mean any local doctor can act on full information.
The price gap has an economic explanation, not a clinical one. Surgical volumes in Istanbul are among the largest anywhere, and delivering world-class hospital care simply costs less in Turkey, so the saving is built into the system rather than carved out of your safety. What you are paying for is a European Board-certified surgeon. What you save would cover your flights many times over.

A final word
There is no hurry, and price is the least useful way to choose. Whoever you are considering, ask them for healed faces, real footage and certificates you can verify. Ours are already on this page. And if surgery isn’t the right answer for your face yet, Dr. Akçay will tell you that too.

Discretion is the default. Exactly two people see your photos: Dr. Akçay and his clinical coordinator. They are never shared, and nothing is ever published without your written consent. Who learns about your surgery is entirely your decision.
